How the Medicare Premium Calculator Works
This calculator provides an estimate of your monthly Medicare premiums based on current 2025 rates. It factors in Part A eligibility, Part B standard and IRMAA-adjusted premiums, optional Part D drug coverage costs, and Medigap supplement premiums adjusted for age and plan type.
The Part B premium is the most variable component, as it is subject to Income-Related Monthly Adjustment Amounts (IRMAA) for higher-income beneficiaries. IRMAA is determined by your modified adjusted gross income from two years prior (2023 income for 2025 premiums). The standard Part B premium of $174.70 applies to individuals earning $103,000 or less.
Medigap premiums in this calculator are estimates based on national averages. Actual premiums vary significantly based on your state, insurer, gender, tobacco use, and the pricing method used (community-rated, issue-age, or attained-age). We recommend getting quotes from at least three insurers in your area for the most accurate pricing.
| IRMAA Income Bracket | Part B Monthly Premium | Part D Surcharge |
|---|---|---|
| ≤ $103,000 | $174.70 | $0.00 |
| $103,001 – $129,000 | $244.60 | $12.90 |
| $129,001 – $161,000 | $349.40 | $33.30 |
| $161,001 – $193,000 | $454.20 | $53.80 |
| $193,001 – $500,000 | $559.00 | $74.20 |
| > $500,000 | $594.00 | $81.00 |
Understanding Each Cost Component
Part A premiums apply only to individuals who did not work long enough to qualify for premium-free Part A. The full 2025 Part A premium is $505/month. If you or your spouse worked 40 or more quarters (approximately 10 years) paying Medicare taxes, your Part A premium is $0.
Part B is required for most Medicare beneficiaries and covers doctor visits, outpatient services, preventive care, and medical equipment. The monthly premium of $174.70 is typically deducted from your Social Security check. Higher earners pay IRMAA surcharges that can more than triple this amount.
Part D drug coverage is optional but strongly recommended. Going without creditable drug coverage results in a permanent late enrollment penalty of 1% per month added to your Part D premium. The average Part D premium in 2025 is $55.50/month, though plans in your area may be higher or lower.
Medigap (Medicare Supplement) insurance is optional and only works with Original Medicare — not Medicare Advantage. Plan G is the most popular option, covering virtually all gaps in Original Medicare except the annual Part B deductible of $240. Premiums range from approximately $100 to $250+ per month depending on age, location, and insurer.
